Oak Longevity Review: Is It Legit?

Oak Longevity review: compounded semaglutide and tirzepatide from $119/mo, whether it names its pharmacy, FDA warning letters and complaints.

Last updated Oct 21, 2026
Quick answer

Oak Longevity is an online GLP-1 program that prescribes both brand-name and compounded medication: compounded semaglutide and compounded tirzepatide. Plans start at $119 a month. We rate it 8.3/10 (Good). It does not name the compounding pharmacy that fills its prescriptions, so the pharmacy's record can't be checked. It ships to all 50 states. Compounded drugs are not FDA-approved. No FDA warning letter is on file for Oak Longevity as of Oct 21, 2026.

Oak Longevity is an online clinic for weight loss, sexual health and longevity care. It suits people who want compounded semaglutide or tirzepatide at a low monthly price, with no video call and no membership fee.

We rate it 8.3 out of 10, tier Good. Semaglutide starts at $119 a month on a 12-month plan, every fee is $0, and it has no FDA warning letter.

The catch: the low prices need a 12-month plan, the month-to-month price shows only at checkout, and Oak does not name its pharmacies.

No FDA warning letter is on file for Oak Longevity in the FDA’s published warning-letter list as of Oct 21, 2026. Oak Longevity doesn't name its compounding pharmacy, so the pharmacy can't be cross-checked.

What is Oak Longevity?

Oak Longevity is a telehealth brand that sells prescription care for weight, sexual health and longevity, delivered to your door. Weight loss is its main line.

Here is how it goes. You pick a treatment plan and fill in a short medical questionnaire. A US-licensed provider reviews it online and prescribes only when it is medically appropriate. No video call is needed. The prescription goes electronically to one of Oak's contracted pharmacies. The drug ships to your door, and refills continue on the same plan.

The weight-loss menu leads with two compounded weekly injections. Compounded semaglutide is the molecule in Wegovy and Ozempic. Compounded tirzepatide is the molecule in Zepbound and Mounjaro. Oak also lists branded GLP-1 pens and oral Wegovy tablets, with the price quoted at checkout. To understand the drugs themselves, read our pages on semaglutide and tirzepatide.

Compounded GLP-1s are prepared by a pharmacy to a prescription, and they are not FDA-approved. Oak serves all 50 states.

Is Oak Longevity legit?

Yes. Oak is a real telehealth brand with US-licensed providers, a stated prescribing process and no FDA warning letter. The gap is the pharmacy: Oak sends prescriptions to contracted pharmacies it does not name, so you cannot check who makes the drug.

Oak describes the pharmacy only in general terms. Its semaglutide page says the drug is "prepared by a licensed US compounding pharmacy for an individual patient's prescription, under state board of pharmacy oversight." With no names, there is nothing to match against the FDA register we read. For a compounded program, where the pharmacy makes the drug itself, that matters.

The home page does not say plainly that the weight-loss drugs are compounded. Each drug page names the molecule and the form. Ask Oak for the pharmacy name before your first order, and look it up on your state board's site.

Compounded semaglutide costs $119 a month on a 12-month plan. Compounded tirzepatide costs $185 a month on a 12-month plan. The drug, the provider and shipping are included. There is no membership fee and no consultation fee. The home page advertises weight loss "from $119/mo", which is the 12-month semaglutide rate.

Shorter plans cost more. Oak sells month-to-month plans and 3-, 6- and 12-month plans. Multi-month plans are billed every three months or in full at checkout. Oak shows the month-to-month price only before you pay, not on a public page. We could not check it, so we do not list one. An older weight-loss page still says programs start at $190 a month.

Oak does not bill insurance. Check your price again at each refill. For scale, a pharmacy pays $871.12 for a Wegovy 1.7 mg pen package, effective September 23, 2026. That is an acquisition cost for the brand-name drug, not what a patient pays.

How does Oak Longevity work?

The care runs online, with no video call. You fill in a health form, and a board-certified provider reviews it. Oak's weight-loss page says a provider "approves your treatment the same day" when it is appropriate.

After that you get free access to the Oak Care Team and free health coaching. Your provider can adjust your dose during treatment. The terms describe a TeleMed consultation with one of the medical professionals Oak contracts with. They add: "If you have taken a lab test through the lab partners, you may discuss the results with the medical Provider you selected."

Oak does not state whether you need labs before you start, or how often follow-ups happen. Ask the care team both questions before you pay.

Is Oak Longevity safe? FDA status and warning letters

Oak Longevity has no FDA warning letter and no open flag on its record. There is nothing against a pharmacy either, because none is named.

The bigger question with any compounded program is the drug itself. Compounded GLP-1s are prepared by a pharmacy to a prescription. They are not FDA-approved: the FDA doesn't review their safety, effectiveness or quality before they're sold, and they are not the same as Wegovy or Zepbound. Formulas can differ from one pharmacy to the next, and some add ingredients such as vitamin B12. Compounding pharmacies are licensed and inspected by state boards of pharmacy; outsourcing facilities (503B) also register with the FDA and are inspected by it. The FDA can issue warning letters when either breaks rules on safety or sterility. Oak asks you to complete a medical questionnaire, and a provider prescribes only when it is medically appropriate.

For side effects, doses and who should not take these drugs, read our semaglutide and tirzepatide pages, which quote the FDA labels.

How to cancel Oak Longevity

Cancellation
Minimum 12 months
Money-back guarantee
None. The terms state that once a provider has prescribed and the order has gone to the pharmacy, it "can no longer be cancelled and is non-refundable".

You can cancel at any time in your patient portal, by email to hello@oaklovesyou.com or by phone at (435) 244-7757. On a monthly plan there is no minimum term. Cancelling takes effect at the end of the billing period you have already paid for, and that period is not refunded. Oak aims to act on a cancellation within 24 hours, and a paid order within 48 hours of shipment still ships. Once a provider has prescribed and the order has gone to the pharmacy, you cannot cancel it and you get no refund.

If a provider does not approve you, you are not charged: the hold on your card is released and any amount collected is refunded in full.

The trap is the multi-month plan. If you cancel a 3-, 6- or 12-month plan that has more than one scheduled payment before its term ends, Oak charges back the discount on the months you received. The charge is the month-to-month price when you bought, minus your plan's monthly price, times the months received. It never exceeds the payments left on your plan, and it does not apply if your provider stops or changes your treatment for medical reasons. If you paid the plan in full, Oak stops future shipments and refunds the months not yet shipped, minus that same discount. The terms give an example: "on a $209/month 12-month plan when the month-to-month price is $299, cancelling after 6 months of treatment results in a charge of $540 (6 × $90)."

So take a longer plan only if you are sure you will stay. If you have side effects, talk to your provider.

What customers say about Oak Longevity

Oak's weight-loss page shows a few short patient quotes, signed with a first name only. We cannot check who wrote them, so there are no quote cards on this page. This site collects no independent reviews either.

That leaves you with little to go on from other patients. Oak is a newer platform, and independent reviews are still thin. Read the prices, the plan lengths and the refund rule above for what you will pay and what you sign. If you look for Oak Longevity reviews elsewhere, check what they say about the price after the quiz and the 12-month plan before you commit.

How we scored Oak Longevity

Our score combines seven checks. Each one counts for a fixed share of the total, listed below. Commission is not an input.

  • Price transparency: 9/10 (counts for 20% of the total score). Every fee is $0, the 12-month prices are on public pages and our latest re-read matched, but the month-to-month price shows only at checkout.
  • Cost and value: 9/10 (counts for 15% of the total score). Rank 2 of 10 in its group, on the $119 12-month semaglutide rate, with no fee load.
  • Medication and pharmacy traceability: 5.8/10 (counts for 15% of the total score). No pharmacy named and no pharmacy type stated; the molecule, the form and compounding are stated.
  • Clinical access and oversight: 7.2/10 (counts for 15% of the total score). Same-day review, no lab policy.
  • Safety and enforcement: 10/10 (counts for 20% of the total score). no FDA warning letter and no open flag.
  • Terms, commitment and refunds: 7.34/10 (counts for 10% of the total score). Monthly plans cancel anytime and the refund rule is stated, but the 12-month plans lock you in and charge back the discount if you leave.
  • Availability and fulfilment: 9/10 (counts for 5% of the total score). All 50 states and free shipping.

We rescored Oak on October 10, on its new 12-month prices. Price transparency rose from 7.67 to 8.84. Terms fell from 10 to 7.34, because the low rates need a 12-month plan. Cost and value held at 10. Traceability fell from 5.8 to 4.6 after the home page changed. The total fell from 8.4 to 8.2. We rescored Oak again on October 16. Price transparency rose from 8.84 to 9, because our latest price re-read matched the prices on record. Traceability rose from 4.6 to 5.8, as our record now credits it for saying plainly that its drugs are compounded. One program left its group, now 10 programs; Oak still ranks 2, but cost and value fell from 10 to 9 in the smaller group. The total rose from 8.2 to 8.3, and our confidence rose from low to medium. The method is on our methodology page.
