MEDVi Review: Is It Legit?

MEDVi reviews and complaints, its open FDA warning letter (February 2026), and the cost after month one: semaglutide $299, tirzepatide $299 to $399 a month.

Quick answer

MEDVi is an online GLP-1 program that prescribes both brand-name and compounded medication: compounded semaglutide, compounded tirzepatide, Wegovy and Zepbound. Plans start at $299 a month. We rate it 6.7/10 (Fair). It names the pharmacies that fill its prescriptions: Triad Rx, RedRock Pharmacy and Beaker Pharmacy & Compounding. It ships to all 50 states. Compounded drugs are not FDA-approved. MEDVi has an FDA warning letter on file, so we do not recommend it.

MEDVi is an online weight-loss program run by MEDVi, LLC, with prescriptions from OpenLoop Health clinicians. It sells compounded semaglutide as a weekly injection or a daily dissolving tablet, compounded tirzepatide, and Wegovy or Zepbound for a membership fee plus the drug.

MEDVi has an open FDA warning letter dated February 20, 2026. The FDA said the site pictured vials labelled "MEDVi", as if MEDVi made the drugs, and claimed the "same active ingredient" as Wegovy, Ozempic, Mounjaro and Zepbound. Both claims are gone from MEDVi's site today, but as of October 24, 2026, the FDA has published no close-out. The safety section explains it.

On MEDVi's main page, compounded semaglutide costs $179 for the first month, then $299 a month, as of October 24, 2026. Tirzepatide is $299 or $399 a month after a discounted first month, depending on the plan page. The price you see depends on the page you land on.

We rate it 6.7 out of 10, tier Fair. It names its prescribing network and its pharmacies, but its month-to-month prices are among the highest we track, its checkout and refund page disagree, and its site still makes weight-loss claims for compounded drugs.

It’s for you if
  • You want compounded semaglutide as a daily dissolving tablet rather than an injection.
  • You want compounded and brand-name options from one program.
  • You want to see which pharmacies a program uses.
It’s not for you if
  • You want a low month-to-month price: after the first month, semaglutide is $299 and tirzepatide $299 to $399.
  • You want a program with no open FDA warning letter.
  • You want a clear refund rule for a multi-month plan.
  • You want to know your clinician's name before you pay.
  • You want insurance billing.
  • You don't want injections, or don't feel comfortable injecting yourself, and the tablets don't suit you.

What is MEDVi?

MEDVi is a telehealth company that sells prescription weight-loss medication online. The company is MEDVi, LLC, at 131 Continental Drive in Newark, Delaware, and its founder and CEO is Matthew Gallagher. MEDVi does not employ the prescribers. Its site says OpenLoop Health, "a network of US-licensed doctors", decides who qualifies, and that "OpenLoop Health clinicians retain the decision to prescribe". It also names "CareGLP Affiliated P.C.s" as a provider of medical treatment.

MEDVi sells compounded semaglutide as a weekly injection or a daily dissolving tablet, and compounded tirzepatide as a weekly injection. It also lists the Wegovy pill, Wegovy injections and Zepbound for a $99 membership plus the cost of the medication. Compounded GLP-1s are prepared by a pharmacy to a prescription, and they are not FDA-approved; the safety section explains what that means.

MEDVi also sells Quad, a men's sexual-performance product with sildenafil and tadalafil that "dissolves under your tongue", on a separate site. This review covers only the weight-loss program. MEDVi is not MEDvidi, a different company, MEDvidi, Inc., which runs an online mental health service.

Is MEDVi legit?

MEDVi has an open FDA warning letter dated February 20, 2026, about how its compounded drugs were marketed. It is a real, operating company: it names its prescribing network and three pharmacies with their addresses, and it has about 15,000 reviews on Trustpilot.

The letter is about marketing. The FDA reviewed medvi.io in December 2025. It objected to product images with "MEDVi" on the label, "suggesting MEDVi is the compounder of those drugs when in fact it is not", and to claims of the "same active ingredient" as Wegovy, Ozempic, Mounjaro and Zepbound, which it said imply FDA approval. In April 2026, MEDVi said medvi.io was run by an affiliate marketing agency; the letter is addressed to MEDVi, LLC at MEDVi's own address.

On MEDVi's own sites today, the vials read "PRESCRIPTION GLP MEDICATION" with no MEDVi name, and we found neither "same active ingredient" claim. The footer says "MEDVi does not produce compounded medications". But the same page still promises results for a compounded program: "Lose an average of 18% of your body weight" and "93% kept the weight off for good". The FDA asked MEDVi to address any "other website claims that could be construed as false or misleading".

MEDVi also has a public record beyond the FDA. In April 2026, reporting after a New York Times profile described AI-generated doctor personas in ads for MEDVi, and MEDVi said it had "recently become aware" of such ads. MEDVi's footer says some site content may be generated with artificial intelligence and that people in its ads "may be actors or models". Five lawsuits naming MEDVi, LLC were filed in federal court in California between November 2025 and April 2026, at least three of them over marketing calls, texts or emails. They are allegations, and none has been decided.

The pharmacies are named with addresses: Triad Rx in Daphne, Alabama, RedRock Pharmacy in St. George, Utah, and Beaker Pharmacy & Compounding in McKinney, Texas. MEDVi does not say which one fills which order.

As of October 24, 2026, MEDVi's compounded semaglutide injections cost $179 for the first month, then $299 a month on its main page. The page says: "Refills are locked in at $299". The dissolving tablets start at $249; the page does not show the price after the first month.

The plan page MEDVi shows after the questionnaire lists a "Fall Special" with a code already applied. There, semaglutide is $159 for the first month, then $299 a month, and tirzepatide is $259 for the first month, then $399 a month. A second plan page offers tirzepatide at $199 for the first month, then $299 a month. The page a link opens decides which of these you see:

  • MEDVi's GLP-1 page: semaglutide $179 for the first month, then $299 a month; tirzepatide not listed.
  • A variant of that page: semaglutide $99 for the first month, then $199 a month; tirzepatide not listed.
  • Plan page after the questionnaire (code FALL140): semaglutide $159, then $299 a month; tirzepatide $259, then $399 a month.
  • Second tirzepatide plan page (code NEWCUS140): tirzepatide $199, then $299 a month; semaglutide not listed.

Prices read on October 24, 2026.

MEDVi charges no membership fee on its compounded plans. Its page says "No membership or hidden fees!", and the price includes the clinician review and the medication, with free shipping. MEDVi says the price is the same at every dose. The brand-name plans are the exception: Wegovy and Zepbound cost "$99 Membership + Medication Cost", and MEDVi does not publish the medication cost. Its terms add that the final charge "may fluctuate contingent upon the prescribed medication and the chosen pharmacy for order fulfillment", and that "medical consult fees are not subject to or eligible for a refund".

Longer plans cost less per month. On the plan page, semaglutide is $475 for a 3-month plan ($158 a month), $919 for 6 months ($153 a month) and $1,799 for 12 months (about $150 a month). The 3-month plan arrives "in a single shipment", and the page offers to split the cost into installments. The checkout says financing such as Klarna is between you and the lender, and that those payments are "generally not refundable by MEDVi". If you have side effects, talk to your provider.

MEDVi does not take insurance: "all MEDVi prescriptions are cash-pay". Its page says "HSA/FSA Approved!".

MEDVi tirzepatide price

As of October 24, 2026, MEDVi's compounded tirzepatide costs $399 a month after a $259 first month on the plan page MEDVi shows after its questionnaire. A second plan page sells it at $299 a month after a $199 first month. MEDVi says the price is the same at every dose. Paid up front, tirzepatide is $769 for a 3-month plan ($256 a month) and $1,519 for 6 months ($253 a month). Compounded tirzepatide is not FDA-approved and is not the same as Zepbound or Mounjaro; the safety section explains it.

What to watch

MEDVi has an open FDA warning letter from February 2026, its prices depend on the page you land on, its site makes weight-loss claims for compounded drugs, and its checkout says multi-month plans are not refunded while its refund page says unshipped months are.

How does MEDVi work?

MEDVi's care is online and starts with a questionnaire. When you finish it, MEDVi shows a "Success Probability" of 94%, before a clinician has seen your answers. You pay for a plan before a clinician reviews you. MEDVi says its providers review every form within 24 hours, and that an OpenLoop clinician meets you after checkout to decide whether you qualify. If you are prescribed, a pharmacy ships the medication to your door, with free shipping.

MEDVi advertises "Unlimited 24/7 support", "1:1 physician guidance" and free dietitian visits and care coaching, led by certified medical assistants and registered dietitians. No clinician is named on the site. The plan page says orders are "Prescribed & shipped within 48 hours". MEDVi's intake lists all 50 states and DC.

The pharmacies are listed in the footer of every page. Which one fills your order, and the type of each, is not stated.

Is MEDVi safe? FDA status and warning letters

MEDVi has an open FDA warning letter to MEDVi, LLC dba MEDVi, dated February 20, 2026. The FDA reviewed medvi.io in December 2025 and found two problems: vial images labelled "MEDVi", which suggested MEDVi makes the drugs, and claims of the "same active ingredient" as brand-name drugs, which implied FDA approval. The letter names no safety, sterility or quality finding. Both claims are gone from MEDVi's sites, but as of October 24, 2026, the FDA has published no response from MEDVi and no close-out. The letter's list of violations was not exhaustive.

Two of MEDVi's pharmacies have FDA records of their own. Triad Rx, at the Daphne, Alabama address MEDVi lists, received an FDA warning letter on February 5, 2019, about insanitary conditions in sterile compounding; the FDA closed it out on May 3, 2023. The FDA issued a Form 483 on July 31, 2026, to Red Rock Pharmacy's site in Springville, Utah, after an inspection that covered semaglutide and tirzepatide lots. MEDVi lists Red Rock's St. George address, so we cannot tell whether that site fills MEDVi orders. We found no FDA record for Beaker Pharmacy.

The bigger question with any compounded program is the drug itself. Compounded GLP-1s are prepared by a pharmacy to a prescription. They are not FDA-approved: the FDA doesn't review their safety, effectiveness or quality before they're sold, and they are not the same as Wegovy or Zepbound. Formulas can differ from one pharmacy to the next, and some add ingredients such as vitamin B12. Compounding pharmacies are licensed and inspected by state boards of pharmacy; outsourcing facilities (503B) also register with the FDA and are inspected by it. The FDA can issue warning letters when either breaks rules on safety or sterility. MEDVi's footer says its compounded medications are "not FDA-approved or evaluated for safety, efficacy, or quality".

What the website claimed

The FDA says the company’s own marketing or labelling was misleading — for example a claim about safety, results or approval, or a label implying the company makes a drug a pharmacy compounds. This is about how the drug was presented, not about the medicine a patient received.

“The compounded semaglutide and tirzepatide products displayed on your website identify “MEDVi” on the pictured label, suggesting MEDVi is the compounder of those drugs when in fact it is not.”

What does an FDA letter mean?

A warning letter asks a company to fix something. It is not a recall, it does not suspend a licence, and it is the FDA’s position rather than a court’s finding.

How to cancel MEDVi

Cancellation
Cancel anytime
Money-back guarantee
No money-back guarantee, despite a "MEDVi guarantee" badge. You get a full refund if you cancel within 72 hours and have received no service, or if a provider disqualifies you. Monthly charges are not refunded. MEDVi's refund page says unshipped months of a multi-month plan are refunded; its checkout says they are not.

You cancel MEDVi by emailing hello@medvi.org or through the chat in your patient portal, at least 72 hours before your next billing date. If you miss that window, the next month is charged and the cancellation takes effect after it.

Refunds are narrow. You get a full refund if you cancel within the first 72 hours and have received no services, or if a provider disqualifies you. A monthly charge is not refunded once the month has started. Elsewhere, the same policy says, in capitals, that no refund is issued upon cancellation of the subscription.

The two documents disagree on long plans. MEDVi's refund policy says a plan "charged 2 or more months at a time" is refunded "for the months for which no medication was shipped". Its checkout says "multi-month packages will not be refunded for unused medication or remaining months" and that "cancellation fees may apply". MEDVi does not state the fee. A 3-month plan ships at once, so little would be refunded under either rule. Only choose a multi-month plan if you expect to stay on the medication that long. If you have side effects, talk to your provider.

Disputes go to arbitration. MEDVi's terms waive class actions; you can opt out by email within 30 days of accepting them.

MEDVi reviews and complaints

As of October 24, 2026, MEDVi has a 4.3 TrustScore on Trustpilot from about 15,000 reviews; 79% are five stars and 12% are one star. MEDVi invites customers to review it, and replies to most negative reviews. The good reviews praise attentive providers and care coaches, quick appointments and on-time delivery.

The one-star reviews describe shipment delays, fewer doses than expected, extra consultations before a refill, charges after shipments stopped and trouble cancelling.

Other review sites score MEDVi lower. Mira, which reviewed MEDVi on September 30, 2026, reported an F rating and 751 complaints on the Better Business Bureau, where MEDVi is not accredited, and 3.3 out of 5 from 2,226 reviews on ConsumerAffairs. Mira also reported customers saying their vials ran 20 to 40 units short. We could not open either site to check.

MEDVi's intake shows "Excellent 4.5"; that is Trustpilot's star graphic, not its 4.3 score. MEDVi's own testimonials are marketing; its footer says images representing them "may use models". This site collects no independent reviews of its own yet.

How we scored MEDVi

Our score combines seven checks. Each one counts for a fixed share of the total, listed below. Commission is not an input.

  • Price transparency: 7.5/10 (counts for 20% of the total score). Prices and what they include are published. We take points off because the price depends on the page you land on and on a code the plan page applies itself, and because the tablet refill price and the Wegovy and Zepbound drug prices are not shown.
  • Cost and value: 2/10 (counts for 15% of the total score). We compare month-to-month prices: MEDVi's $299 for semaglutide ranks 15 of 16 compounded programs, against a median of $199.50. The first-month and multi-month prices are lower but are not in this comparison.
  • Medication and pharmacy traceability: 7.6/10 (counts for 15% of the total score). Three pharmacies named with addresses, but not which one fills an order or what type each is.
  • Clinical access and oversight: 8.3/10 (counts for 15% of the total score). A named prescribing network, a stated review time and included messaging, but no named clinician.
  • Safety and enforcement: 5/10 (counts for 20% of the total score). An open FDA warning letter to MEDVi, and a closed-out 2019 letter to Triad Rx. We take a further point off because the letter has no close-out and the site still makes weight-loss claims for compounded drugs.
  • Terms, commitment and refunds: 10/10 (counts for 10% of the total score). Cancel any time with 72 hours' notice. Note that the checkout and the refund policy disagree on multi-month plans, and the checkout mentions a cancellation fee it does not state.
  • Availability and fulfilment: 10/10 (counts for 5% of the total score). MEDVi's intake lists all 50 states and DC, and shipping is free.

This is the first score for MEDVi. Confidence is low: MEDVi's pages fill in their prices by script, so our automatic re-read could not confirm them; we read them in a browser on October 24, 2026. Two of the seven checks carry an editor's adjustment, explained above; together they lower the total by 0.4. The method is on our methodology page.
